COMPOSITION—_Dimenhydrinate .50mg —__.Excipients: Avicel, Magnesiu stearate, Sodium starch glycolate, Aerosil, Methocel, Ethyl cellulose, PEG 6000, Titanium dioxide, Blue patent V…………….– sqf{ filmcoated tablet. INDICATIONS— Prevention andtreatment ofnausea, vomiting, vertigo associated with motion sickness — Prevention and treatment ofvomiting andvertigo associated with Méniére’s disease andother vestibular disturbances. DOSAGE AND ADMINISTRATION — Prevention andtreatment ofnausea, vomiting, vertigo associated with motion sickness. s_The usual dose ofadults and children over 12 years old: 1-2tablets, 3-4times daily, not exceeding 400mgdaily. e Prevention ofmotion sickness: thefirst dose should begiven atleast 30minutes before travelling. e Children 6to12years old:%- 1tablet every 6to8hours toamaximum of150mg daily — Treatment ofMéniére’s disease: %- 1tablet, 3times daily. CONTRAINDICATIONS— Hypersensitivity todimenhydrinate ortoother antihistamines. — Children under 2years old. – Close-angle glaucoma. — Urinary retention involved totheurethro-prostatic problems. PRECAUTIONS— Avoiding the use ofalcoholic drinks orother CNS depressants during treatment of dimenhydrinate. — Chronic constipation, urinary bladder obstruction, benign prostatic hyperplasia. – Elderly DRUG INTERACTIONS — Drugmayenhance theeffects ofCNS depressants, such asalcohol and barbiturates. ~— Drugmayenhance theeffects ofanticholinergic agents. — Concomitant use ofdimenhydrinate with aminoglycoside antibiotics orother ototoxic agents may mask theearly symptoms ofototoxicity. USE INPREGNANCY AND LACTATION — Drug should notbeused during pregnancy only when clearly needed. = Drug should notbeused inbreast-feeding woman. EFFECTS ONABILITY TODRIVE OROPERATE MACHINERY Donot usewhen driving oroperating machinery. ADVERSE REACTIONS — Drug may cause somnolence, headache, dizziness, incoordination, blurred vision, drymouth and respiratory system, tinnitus. — Rare cases ofanorexia, constipation or diarrhoea, urinary difficulty and retention, palpitation and hypotension have been report. — Paradoxical stimulation may occur very rarely inchildren ortheelderly such asexcitement, tremors, insomnia, convulsions. *Contact your physician ifyou experience any adverse reactions while using this medicine. (ERDOSAGESymptoms:e Clinial manifestation ofoverdosage ofchildren may include dilated pupils, flushing, hyperpyrexia, excitement, hallucinations, confusion, ataxia, convulsions, coma, respiratory failure, cardiovascular collapse. © Overdosage ofadult may cause difficult speaking and difficult swallowing, psychoses. — Treatment: There isnospecific antidote forantihistamines. Hospitalization forsymptomatic and supportive treatment. PHARMACODYNAMICS Dimenhydrinate, anethanolamine-devivative, isasedating antihistamine. Itdiminishes or abolishes themajor actions ofhistamine inthebody bycompetitively antagonizes theeffects of histamine onH,-receptors. Italsohas anticholinergic, antiemetic andsedative effects. PHARMACOKINETICS Dimenhydrinate iswell absorbed from thegastrointestinal tract. The antiemetic effect appears 15-30minutes after oral administration. The duration ofactions ranges from 3-6hours. Dimenhydrinate iswidely distributed throughout thebody and crosses theplacenta. Drug is metabolized intheliver and excreted inurine. Asmall amount ofdrug issecreted inbreast milk. TƯƠNG TÁC THUỐC ~__. Thuốc cóthểlàmtăng tácdụng cácthuốc ức chếthần kinh trung ương như rượu, vàbarbiturat. ~ Thuốc cóthểlàmtăng tácdụng cácthuốc kháng cholinergic ~_ Dùng cùng vớithuốc kháng sinh nhóm aminoglycosid hoặc các thuốc độc đốivớitaikhác, dimenhydrinat có thể làm che lấpcáctriều chứng sớm của độctínhởtai. QUÁ LIỀU VÀXỬTRÍ —_Triệu chứng: «_Quá liểu ởtrẻemcóthểxảy ragiãn đồng tử,đỏmặt, sốt cao, kích động, ảogiác, lúlẫn, mấtđiều hoà, cogiật, hôn mê, suyhôhấp, truy timmạch s_Quá liểuởngười lớncóthểgâyrakhó nóivàkhónuốt, loạn tâm thần. ~__ Xửtrí:Không cóthuốc giảiđộc đặc hiệu các thuốc histamin. Chuyển ngay đến bệnh viện để điểutrịtriệu chứng vàhỗtrợ. DƯỢC LỰC HỌC Dimenhydrinat làthuốc kháng histamin, anthần gây ngủ, thuộc nhóm dẫn xuất ethanolamin. Thuốc kháng histamin làm giảm hay mất những tácđộng chính củahistamin bằng cách cạnh tranh với histamin ởthụthểH,.Dimenhydrinat cũng cótácdụng kháng cholinergic, chống nôn vàtác dụng anthần. DƯỢC ĐỘNG HỌC Dimenhydrinat được hấp thutốtqua đường tiều hóa. Tác dụng chống thường 15-30phút sau khiuống. Thời gian tácdụng củathuốc 3-6giờ. Thuốc cóthểđược phần bốrộng rãi vào các tổchức của cơthể, qua được nhau thai, bịchuyển hoá ởgan vàthải trừqua nước tiểu. Một lượng nhỏthuốc được phân bổvào sữamẹ. ĐỂXA TẦM TAY CỦA TRẺ EM.
