THÀNH PHÀN: Mỗi viên 5 —Hoạt chất: Aceclofenac .. : .100mg —Táđược: Cellulose vìtỉnh thể PH‘101, ‘colloidal silicon dioxid, low substituted hydroxypropyl cellulose, natri starch glycolat, magnesi stearat, polyethylen glycol 6000, hypromellose 2910, talc, titan oxid. DANG BAO CHE: Vién bao phim QUY CÁCH ĐÓNG GÓI: Hộp 10vix10viên CHỈ ĐỊNH Aceclofenac làmột thuốc kháng viêm không steroid (NSAID) được chỉ định đểgiảm đau vàkháng viêm trong điều trịthoái hoá khớp, viêm khớp dạng thấp, viêm đốt sống cứng khớp. LIỀU LƯỢNG VÀ CÁCH DÙNG Nên uống thuốc cùng với thức ăn hoặc ngay sau khi ăn. —Người lớn: Liều khuyến cáo là100 mg x2lần/ngày. —.Swy gan: Giảm liều khởi đầu xuống 100 mg/ngày. CHONG CHi ĐỊNH —Quá mẫn vớiaceclofenac, aspirin, các NSAID khác hoặc với bất kỳthành phần nào của thuộc. — Viêm loét dạ dày tiến triển, tiền sử viêm loét hoặc chảy máu đường tiêu hoá.
Suy thận từtrung bình đến nặng. Bệnh nhân đang nhiễm khuẩn. Không nên sửdụng trong thời gian mang thai, đặc biệt 3tháng cuối thai kỳ. SỬ DỤNG THẬN TRỌNG —Thận trọng khi sửdụng cho người bệnh mắc các bệnh vềđường tiêu hoá, viêm loét ruột, bệnh Crohn, bất thường về máu, porphyria gan. -Tiên sửhen suyễn. —_Tiền sửsuy tìm hoặc hạ huyết áp. —Suy gan nhẹ, suy tim nhẹ, suy thận nhẹ. —Thận trọng khi sửdụng ởngười già. —Thận trọng sửdụng trên phụ nữđang cho con bú vìAceclofenac cóthể hiện diện trong sữa mẹ ởnông độrat thấp, Nó ĐỘNG LÊN KHẢ NĂNG LÁI XE VÀ VẬN HÀNH MÁY MỘCCác tác dụng phụ như chóng mặt, buồn ngủ, mệt mỏi hay rối loạn thị giác cóthể xảy ra. Nên thận trọng khi láixehay vận hành máy moc.SU DUNG OPHY NU CO THAI VA CHO CON BÚ -ViNSAID cé kha nang tac dong lên hệtim mạch của thai nhỉ (nguy cơđóng sớm ống động mạch), nên bịchống chỉ định ởba tháng cuối thai kỳ. Trong khí sinh, NSAID cóthê làm chậm các cơn cothắt tửcung và kéo dài quá trình sinh, dẫn đến nguy cơ chảy máu ởngười mẹ và đứa trẻ. Không nên sửdụng NSAID trong 3 tháng đầu vàgiữa thai kỳ hay khi đau đẻ trừ khí lợi ích đối với người mẹ cao hơn nguy cơcóthẻ đối với thai nhỉ. —Trong các nghiên cứu cóhạn, các NSAID cóthể hiện diện trong sữa mẹ ởnồng độrấtthấp. Vì vay, tránh sửdụng NSAID ởngười me dang cho con búnếu cóthê. TƯƠNG TÁC THUÓC —Lithi, digoxin: tăng nông độ huyết tương của lithi vàdigoxin. —Thuốc chống đông: tăng tác dụng chống đông. —Methotrexat: tng nông độ huyết tương của methotrexat, dẫn đến tăng độc tính khi phối hợp NSAID trong vòng 24 giờ sau khi uống metrotrexat. —Ciclosporin: tăng độc tính trên thận của ciclosporin. —Thuốc hạáp: giảm tác dụng hạáp. -Taube lợitiểu: giảm tác dụng lợi tiểu, tăng độc tính trên thận của NSAID.

Theo dõi kali huyết khi sửdung chung với các thuốc lợitiểu tiếƒ kiệm kali hoặc các thuốc ứcchế men chuyền. -Corticosteroid, NSAID khác, thuốc kháng tiểu cầu, thuốc ứcchế taihap thu chon loc serotonin (SSRI); tang nguy co loét vachay máu đường tiêu hoá. —Quinolon: tăng nguy cơđộng kinh. —Mifepriston: không nên dùng NSAID sau khi dùng mifeprison 8 —12ngày viNSAID cóthể làm giảm tác dụng của mifepriston. —Tacrolimus: ting nguy co déc thận. —Zidovudin: tang nguy cơ độc máu. —Ritonavir: ting néng d9aceclofenac trong huyét tuong. Nồng độ đỉnh trong huyết tương đạt được sau l,25 —3gid. —Aceclofenac gắn kết chặt chẽ với protein (trên 99%). Phân bố vào hoạt dịch với nồng độ khoảng 57% sovới nồng độ trong huyết tương. Thể tích phân bốkhoảng 25L. —Thời gian bán thải trung bình khoảng 4giờ. Aceclofenac trong huyết tương chủ yếu ởdạng không đổi. Chất chuyển hoá chính được tìm tháZetrong huyết tương là4′-Hydroxyaceclofenac. Khoảng %ligu ding được thải trừ qua thận, chủ yếu ởdạng. hydroxy hoá. —Các tínhchất dược động của aceclofenac không đổi ởngười giả. QUA LIEU VA XỬ TRÍ -Trigu chimg: dau dau, buén nén, nén mira, dau thugng vi, xuat huyết tiêu hoá, hiếm khi tiêu chảy, mất phương hướng, kích động, hôn mê, buồn ngủ, chóng mặt, ùtai, choáng, thỉnh thoảng động kinh. Trường hợp bịngộ độc rõrệt, suy thận cấp vàsuy gan cóthể xảy ra. —Xử trí:Điều trịtriệu chứng vàđiều trị hỗtrợ. Trong vòng |giờ sau khi uống một lượng thuốc cóthể gây ngộ độc, rửa dạ dày và chữa trịbằng than hoạt. Đảm bảo thông tiểu tốt, theo đõi chặt chẽ chức năng gan, thận vàgiám sát bệnh nhân trong ítnhất 4gid. Nếu cóđộng kinh, cóthể tiêm tĩnh mach diazepam. Gây lợi tiêu, thẩm phân hoặc lọc máu không có hiệu quả trong việc loại bỏ thuốc rakhỏi cơthể. COMPOSITION: Each film coated tablet contains: —Active ingredient: Aceclofenac………………………………. 100 mg ~-Expicients: Microcrystalline cellulose PH 101, colloidal silicon dioxide, low substituted hydroxypropyl cellulose, sodium starch glycolate, magnesium stearate, polyethylene glycol 6000, hypromellose 2910, talc, titanium oxide. DOSAGE FORM: Film-coated tablet. HOW SUPPLIED: H6p 10vix10viên, INDICATIONSAceclofenac, anonsteroidal anti-inflamatory drug (NSAID), is indicated for the relief ofpain and inflammation inosteoarthritis, theumatoid arthritis and ankylosing spondylitis. DOSAGE AND ADMINSTRATION Should betaken with food or immediately after meals. —Adult: 100 mg twice aday. —Hepatic impairment; Reduce initial dose to100 mg daily. CONTRAINDICATION~Hypersensitivity to aceclofenac, aspirin, NSAIDs or to any component ofthe drug. —Severe hepatic and cardiac failure. ~Moderate tosevere renal impairment. -Active orprevious peptic ulcer, history of gastrointestinal (GI) bleeding.—Patients with infections. -Should not beprescribed during pregnancy, especially during the last trimester ofpregnancy. Becareful driving oroperating machinery PREGNANCY AND BREASTFEEDING —Inview of the known effects ofNSAIDs onthe foetal cardiovascular system (risk ofclosure ofthe ductus arteriosus), use inthe last trimester ofpregnancy iscontraindicated. The onset oflabour may bedelayed and the duration increased with anincreased bleeding tendency inboth motherand child. NSAIDs should not be used during the first two trimesters ofpregnancy orlabour unless the potential benefit tothe patient outweighs the potential risk tothe foetus.—Inlimited studies, NSAIDs can appear inbreast milk invery low concentrations. NSAIDs should, ifpossible, beavoided when breast- feeding.DRUG INTERACTION ~Lithium, digoxin: Aceclofenac may increase the plasma concentrations oflithium and digoxin. ~Methotrexate: NSAIDs may increase methotrexate plasma levels, resulting inincreased toxicity when NSAIDs are administered within 24hours ofmethotrexate administration. —Ciclosporin: Increased ciclosporin nephrotoxicity. —Anti-hypertensives: Reduced anti-hypertensive effect. —Diuretics: Reduced activity of diuretics, increased risk of nephrotoxicity ofNSAIDs, —Anticoagulants: Aceclofenac may enhance the activity of anticoagulants.

-Monitor scrum potassium when used with potassium-sparing diuretics and ACE inhibitors. —Corticosteroids, other NSAIDs, anti-platclet agents and selective serotonin reuptake inhibitors (SSRIs): Increased risk of GI ulceration orGIbleeding. —Quinolones: Increased risk ofconvulsion. —Mifepristone: NSAIDs should not be used for 8-12days after mifepristone administration asNSAIDs can reduce the effect of mifepristone.—Tacrolimus: Possible increased risk ofnephrotoxicity. -Zidovudine: Increased risk ofhaematological toxicity. —Ritonavir: plasma concentration ofAceclofenac possibly increased byritonavir. SIDE EFFECTS —Common: dyspepsia, abdominal pain, nausea, diarrhoea; dizziness; hepatic enzymes increased. ~Less common: flatulence, gastritis, constipation, vomiting, ulcerative stomatitis; vertigo; pruritus, rash, eczema, dermatitis, urticaria; BUN increased, blood creatinine increased. -Rare: anaemia, anaphylactic reaction, visual disturbance, dyspnoea, melaena, face oedema. Inform doctors with side effects when using medicine. PHARMACODYNAMIC PROPERTIES Aceclofenac, aphenylacetic acid derivative, has antiinflammatory and analgesic properties. Itisapotent inhibitor ofcyclo-oxygenase which is involved inthe production ofprostaglandins. PHARMACOKINETIC PROPERTIES -After oral administration, aceclofenac israpidly and completcly absorbed as unchanged drug. Peak plasma concentrations are reached approximately 1.25 to3.00 hours following ingestion. -Aceclofenac is highly protein- bound >99%). Aceclofenac penetrates into the synovial fluid, where the concentrations reach approximately 57% ofthose inplasma. The volume ofdistribution is approximately 25L. ~The mean plasma climination half-life is around 4hours. Aceclofenac circulates mainly as unchanged drug. 4’- Hydroxyaceclofenac isthe main metabolite detected inplasma. Approximately two- thirds ofthe administered dose isexcreted via the urine, mainly ashydroxymetabolites. ~No changes inthe pharmacokinetics of aceclofenac have been detected inthe elderly. OVERDOSE AND TREATMENT —Symptoms: headache, nausea, vomiting, epigastric pain, gastrointestinal bleeding, rarely diarrhoea, disorientation, excitation, coma, drowsiness, dizziness, tinnitus, fainting, occasionally
convulsions. Incases ofsignificant poisoning acute renal failure and liver damage are possible. -Treatment: Treatment issupportive and symptomatic. Within one hour ofingestion ofapotentially toxic amount, gastric lavage and treatment with activated characoal. Ensure good urine output, monitor closely renal and liver function, observe patient foratleast 4hours. Frequent orprolonged convulsions should betreated with intravenous diazepam, Diuresis, dialysis orhacmoperfusion unlikely tobeuseful. RECOMMENDATION—Keep out ofreach ofchildren. -This drug isused only bydoctor’s prescription. —Read carefully the leaflet before use. —Contact your doctor for further information.
